Case Management Services

Medicaid | OPI Case Management 

Case Management services are available through Medicaid, Oregon Project Independence, and Aging and Disability Resource Connection (ADRC) / Senior Connections. There are many options for Case Management services and a Case Manager will work with you to determine the program that best meets your needs and situation.

Case Managers can help older adults and adults with disabilities who need personal care assistance to access the services and resources they need. Services include:

  • A visit to your home to assess your needs and develop a care plan
  • Referral for other community-based services as needed
  • Arrangement for you to receive supportive services such as home-delivered meals or transportation to medical appointments
  • Regular contact with you to ensure that the services you are receiving continue to meet your needs

Options Counseling

Options Counseling is a free service provided though the ADRC to all seniors and people with disabilities (18 and over), or their family members, to help them plan for their long-term care needs. These services are provided by telephone or in person, which includes home visits. If you would like to speak with an Options Counselor, please call 541-682-3353.
